Please inform us immediately if your
registered Card is lost or stolen, by filling in the form below or by phone (360/379-4000).
If you don't know your serial number, but have previously registered it, enter
as much information as possible and we'll look it up. Once reported as
lost or stolen, we can "lock out" your Card so that it becomes void, and we will
replace your Card (and any remaining fund balance) for a $3.00 fee to cover the
cost of processing and the replacement Card (we will deduct the replacement fee
from your Card balance).
Unregistered Cards may also be replaced, but you must
supply us with the complete serial number and approximate remaining balance, so
that we can positively identify your missing Card.
Please note: once a Card is locked out, it cannot be
reactivated later, so be sure it's really gone before reporting it as

We take your privacy seriously, so we promise to keep your Card registration information confidential and will never sell, rent, loan, trade or otherwise transfer any of your information to others.
To report your Card as lost or stolen,
